Our vision is to create a place that feels like home—where every person is known, valued, and cared for as part of a family. We are currently preparing for licensure with the Minnesota Department of Health and look forward to welcoming residents once approval is granted.

Planned Supportive Services

Our community is designed to offer supportive services in a calm, residential setting that promotes comfort and familiarity. Planned services may include:

Assistance with activities of daily living supports individuals in completing essential self-care tasks needed for everyday life, such as bathing, dressing, eating, toileting, and mobility. This assistance helps promote safety, dignity, and independence while accommodating physical, cognitive, or health-related limitations. Providing appropriate support with daily activities enhances quality of life and allows individuals to function as comfortably and confidently as possible.

A supportive memory care environment is designed to meet the unique needs of individuals experiencing memory loss or cognitive decline. It emphasizes safety, structure, and familiarity while promoting comfort, dignity, and emotional well-being. Through consistent routines, clear cues, compassionate care, and engaging activities, a supportive memory care environment helps reduce anxiety, encourage independence, and enhance quality of life.

Individualized service planning is a person-centered approach that tailors care and support services to each individual’s unique needs, preferences, goals, and abilities. It involves collaboration among the individual, family members, and care providers to develop and regularly update a plan that promotes independence, safety, and overall well-being. This approach ensures services remain flexible, responsive, and aligned with the individual’s changing needs.

Medication assistance, as permitted by licensure, involves supporting individuals with the safe and accurate use of prescribed medications in accordance with regulatory guidelines. This may include reminders, assistance with administration, monitoring for side effects, and proper documentation, while respecting each individual’s rights and care plan. Providing medication assistance helps promote health, safety, and adherence to treatment while ensuring compliance with licensure requirements.

24-hour supervision within a home-like setting provides continuous support and oversight to ensure safety, comfort, and well-being at all times. This approach offers around-the-clock staff availability while maintaining a warm, familiar, and residential atmosphere rather than an institutional environment. Continuous supervision helps address immediate needs, prevent risks, and promote peace of mind for individuals and their families.
